Home Based Geriatric Care

Home visits are an important component of community-based healthcare, enabling healthcare professionals to assess patients in their home environment and provide holistic care. These visits facilitate the identification of medical, functional, social, and environmental issues that may affect patients’ health and well-being. Every Tuesday home visits are conducted to provide comprehensive patient care and support to caregivers.

Objective

To provide comprehensive, holistic, and patient-centred geriatric care through regular home visits by a multidisciplinary healthcare team, with the aim of assessing and managing medical conditions, evaluating functional and environmental factors, supporting caregivers, and enhancing the overall health, safety, and quality of life of elderly individuals within the community.

Team

The home visit team consisted of Consultant of Family Medicine, Family Medicine Senior Registrars, Family Medicine Registrars, CFM Demonstrators, Nursing Officer, Physiotherapist, Medical Students, and Community Health Worker.

Activities

A minimum number of eight (08) households are visited in a month. Patients are assessed for non-communicable diseases, acute medical complaints and appropriate management will be provided. Vital parameters, including Blood Pressure (BP) and Random Blood Sugar (RBS), are checked and documented.

Comprehensive assessments are carried out to evaluate patients’ overall health status, functional abilities, and care needs. In addition, caregiver burden assessments are performed to identify challenges faced by family members and caregivers.

Environmental assessments of the homes are also conducted to identify potential risks and barriers affecting patient safety, mobility, and independence.

 

Medical Student Participation

Medical students actively participated in all home visits. They are involved in patient assessments, health education activities, environmental evaluations, and discussions regarding patient management under the supervision of the healthcare team.

Health Education and Advice

Patients and caregivers are provided with health education and counseling. Advice is given regarding environmental modifications to improve safety and accessibility within the home. Guidance is also provided to caregivers on strategies to reduce caregiver burden and enhance the quality of care provided to patients.
